How to Refer a Patient
Through diagnosis, treatment, and recovery, Institute physicians collaborate with referring colleagues to provide timely and comprehensive consultations, second opinions, and medical and surgical intervention. Because orthopedics and neuroscience are "all we do," patients experience maximum convenience and minimal downtime. And as a referring physician, you'll be active in the care process, with treatment plans and recommendations for discussion and follow-up care.
To schedule a stereotactic radiosurgery consultation, second opinion, or evaluation for your patient, please call the Neurologic & Orthopedic Institute Call Center: (773) 250-0000.
For your patient to be evaluated, you will need to provide:
- Most recent, complete patient history and results from clinical and physical exams.
- Imaging studies conducted within the last 30 days (for mestastes) or forty-five days for functional disorders. You can schedule MRIs at NOHC if necessary.
- Results from previous surgeries and treatments (if applicable).
